December 2013 INSTRUCTIONS FOR OBTAINING A DUI/ALCOHOL EVALUATION At the bottom of these instructions are the names and contact information of facilities licensed to conduct alcohol evaluations in this Judicial Circuit. Please be aware there is a difference between an alcohol evaluation and a licensed DUI evaluation. You should call the alcohol evaluator to schedule an appointment. Before the date of your appointment, you should go to the local driver’s license facility and obtain a copy of your alcohol related driving record. This record can be obtained from the Hearing Officer at the Bloomington Driver’s license facility on any Tuesday, Friday or Saturday. Dates and times may vary at other facilities You will need to take a copy of your Notice of Summary Suspension (which was given to you by the police officer) to your alcohol evaluation – not the Confirmation of Suspension which you received in the mail from the Secretary of State’s Office. The Notice of Summary Suspension which the officer gave you indicates your blood alcohol concentration if you submitted to chemical testing.
