Change of Name/Address Form: If you should not be receiving the tax bill or would like your bill sent to a different name/address, fill out this section and return to the Supervisor of Assessments (P.O. Box 2400 Bloomington, IL 61702 or [EMAIL REDACTED]). Only the Property Owner, Power of Attorney or Trustee can make a change of name or address.
