ORDER FOR REPORTING ON WARD On was appointed as plenary guardian of the person, the estate, the person and estate, of the above named disabled person. THIS COURT HEREBY ORDERS that: 1. The plenary guardian of the person shall file a report as required by 755 ILCS 5/11a-17(b) within thirty days after the expiration of one year from the date of this order, and annually thereafter, or shall appear before the court when ordered to do so for failure to file reports. 2. The plenary guardian of the estate shall present to the Court: a. A verified inventory as required by 755 ILCS 5/14-1 within sixty days from the date of this order, or shall appear before the Court when ordered to do so for failure to file reports; and b. A verified account as required by 755 ILCS 5/24-11 within thirty days after the expiration of one year from the date of this order, and annually thereafter, or shall appear before the Court when ordered to do so for failure to file reports. 3. Upon death of the ward, the plenary guardian shall file a final report as required by 755 ILCS 5/11a-17 and seek final discharge of the guardianship. JUDGE
