Executive Summary This amendment would reallocate $77,800 from the Masada Men’s Treatment Facility to the Joint Intervention Program (JIP). Masada House informed the City of Farmington on July 18, 2014 that they would not be able to utilize the funds to remodel a Men’s treatment facility. The letter states that Masada House is focused on getting the Women’s treatment facility running smoothly and does not currently have the capacity to utilize the funds at this time. The Joint Intervention Program has already been approved to receive $241,908 in CDBG capital funds from the 2014 Annual Action Plan. This amendment will increase their available funds to $319,708. This increase in funds would further increase the capacity of the Joint Intervention Program to construct necessary infrastructure. The funding will be used to create 40 emergency shelter beds and 40 transitional living facility beds. This project will target homeless persons that are utilizing the services through the Joint Intervention Program. 2. Annual Action Plan Amendments Section 91.505 of the HUD regulations provide the City of Farmington the ability to determine what constitutes a minor or substantial amendment to the Annual Action Plan. Criteria for substantial amendment. The jurisdiction shall identify in its citizen participation plan the criteria it will use for determining what constitutes a substantial amendment. It is these substantial amendments that are subject to a citizen participation process, in accordance with the jurisdiction's citizen participation plan. (See Sec. Sec. 91.105 and 91.115.) Section Two on P.7-8 of the Citizen Participation Plan for the 2014-2018 Consolidated Plan defines what qualifies as a Substantial Amendment. A Substantial Amendment to the Annual Action Plan shall constitute any budget adjustment of $25,000 or more or the creation of a new Annual Action Plan project. The Substantial Amendment to the Annual Action Plan process will involve the following steps: • November 9, 2014-Public Notice of the Draft Substantial Amendment, being available for public review and comment. • November 9, 2014-Public Hearing Notice for adoption of the Draft Substantial Amendment. • November 18, 2014-Public Hearing Notice to accept public comments and adopt the Draft Substantial Amendment by resolution. • December 9, 2014-Public Hearing to accept public comments and adopt the Draft Substantial Amendment by resolution. • December 9, 2014-All comments received during the Draft Substantial Amendment public review process will be incorporated into the Final Substantial Amendment. • December 12, 2014-The Final Substantial Amendment will be made available to the public and submitted to HUD. 3. Affected 2013 Action Plan The 2013 Annual Action Plan budgeted $77,800 for Masada House to remodel an existing residential structure for a men’s transitional care facility. The intended outcomes of the capital expenditure were to provide a transitional housing facility in order to treat male homeless clients suffering from substance abuse issues. Available Funds 91.220 91.220 There are no changes in the total available funds as the $77,800 would be redirected to the Joint Intervention Program (JIP). This would increase the CDBG funds available to the Joint Intervention Program from $241,908 to $319,708, including funds from the 2014 Annual Action Plan. 6. Summary of Priorities and Objectives 91.220(C) This Action Plan Amendment does not create any new projects. All projects still meet the National Objectives and the Consolidated Plan Objectives. 7. Expenditure Limits 570.206, 570.201 and 570.208 Masada House and Joint Intervention Program are both capital projects. The reallocation of funds will not impact the maximum caps on planning and administration of 20% and the 15% cap on public service projects. 8. Public Participation 91.105(a) All applicable citizen participation requirements set forth in the 2014-2018 Consolidated Plan’s Citizen Participation Plan will be met or exceeded for the 2013 Annual Action Plan Amendment process. The 2014-2018 Consolidated Plan’s Citizen Participation Plan provides comprehensive information and a high-level of inclusiveness which is why it is being utilized as a guide for this amendment. ---PAGE BREAK--- 5 The opportunity for language, hearing, and accessibility assistance will be part of the public notification process and the 2013 Annual Action Plan Amendment will be available for public review at the Public Library, City Clerk’s office and the Community Development Department. In addition, the Amendment will be posted on the City of Farmington web site. The notices for public review of the Draft 2013 Annual Action Plan Amendment and for the planned adoption will be posted at the Farmington Indian Center, the Sycamore Park Community Center, and the San Juan Center for Independence. The public hearing notice will also be published in English and Spanish and the hearing will take place in an ADA compliant facility. Project Summary 91.220(c) The following worksheet describes the selected project and documents the determinations on how the proposed project meets the HUD eligibilities and Federal objectives. ---PAGE BREAK--- 6 Project Summary Applicant’s Name: Joint Intervention Program (JIP) Goals Supported: Public Facilities Needs Addressed: Homelessness Project Title: Joint Intervention Program Shelter and Transitional Living Facility Project Description: Increase the capacity of the Joint Intervention Program by build necessary infrastructure to help clients. The funding would be used to provide a facility with 40 emergency shelter beds and 40 transitional living facility beds. The Joint Intervention Program Shelter would be an overnight shelter for persons utilizing the Joint Intervention Program that is a joint effort by Presbyterian Medical Services, San Juan Regional Medical Center, Totah Behavioral Health, City of Farmington Police Department, City of Farmington Municipal Court and Four Winds Recovery. Participants would have access to a wide array of services including mental health, substance addiction treatment, medical and dental care, employment and educational support, case management and traditional healing, as many of the participants are Native American.
