Booths issued on first come, first served basis. $125.00 – Artist booth price beginning September 4 $100.00 - Artist booth price if received between August 2 through September 3 $75.00 - Early Bird price – Must be RECEIVED by August 1 If you require more than the allotted space, then you need to purchase an adjoining booth. If you will be sharing a booth applications and photos must be turned in for each artist at the same time. In the space provided below please provide a detailed artwork description for EACH CATEGORY OF ART TO BE SOLD at the 2015 Totah Festival. List materials, tools and techniques used. Note materials that are hand- crafted and those that are pre-purchased. Specify types of metal, stone and or clay. A typewritten description can also be attached to your application. ARTIST HALLMARK Please provide your Hallmark below or attach your hallmark card / statement of authenticity. Your Hallmark is required on every item at your booth and must be on file with Totah Festival. If unable to sign artwork (weaving/ beadwork) a hallmark card and statement of authenticity must be attached to each piece sold. ---PAGE BREAK--- 2015 Totah Festival Artist Application ARTIST BIOGRAPHY Each artist must be juried into the Festival. Please attach or provide your biography in the space provided. Biography should be no more than 100 words and should contain information about your career as an artist, background, training and accomplishments. RELEASE POLICY The Totah Festival Foundation is occasionally contacted by buyers or art organizations looking for a certain type of artwork or an artist from a previous show. Please indicate your wishes with a check mark and initials, only legitimate inquiries are considered, we are very restrictive of our mailing list. Please indicate if images taken by the Totah Festival Photographers can be used for the festival website or promotional purposes. Photographs can be taken and used as describe Do not use images of me or my artwork TOTAH FESTIVAL DRAWING DONATION The Totah Festival Foundation holds a raffle every year with donated items from all participating artists. Items donated are showcased in the Lobby with artist’s business card. Raffle funds cover expenses related to producing the Festival. Donated items are required from each artist and should be brought to check-in Saturday Morning. *Please list the item being donated for the drawing: WAIVER / AGREEMENT Please read the following information. Your signature below indicates your agreement to comply with the following terms and statements: I understand that I must exhibit and sell only MY OWN HANDMADE work. I agree to comply with the Federal Indian Arts and Crafts Act and the New Mexico Indian Arts and Crafts Sales Act for proper representation of my work. Please note the following excerpt from the New Mexico Indian Arts and Crafts Sales Act. It is unlawful for any person to: A. Sell or offer for sale any products represented to be Indian Handmade or Authentic Indian Arts and Crafts unless such products are in fact Indian handmade or authentic Indian arts and crafts. B. Sell or offer for sale any products represented to be Indian Crafted unless such products are in fact Indian Crafted. C. Represent that any Indian arts and crafts product is made of a material, including natural material, unless it is made of that material. D. Fail to disclose in writing that any Indian Arts and Crafts product is made of treated material. E. Prepare, disseminate or otherwise engage in any unfair or deceptive trade practice, including any false, misleading or deceptive advertising or any unconscionable trade practice. By applying to exhibit at the Totah Festival, applicant agrees that they will not utilize the name “Totah Festival” for any purpose without the express written consent of the Totah Festival Foundation. Any request to utilize the name “Totah Festival” shall be made in writing, shall set forth the manner and purpose of such usage and shall be submitted in the same manner as this application. Artist Agreement I understand that if I participate in Totah Festival, the Totah Festival Foundation has the right to verify that I am complying with Totah Festival regulations. My signature constitutes permission to verify any information appearing in this application and signifies that I have read, understand and will abide by the terms set forth in the 2015 Totah Festival Market Terms and Conditions.
