VENDORS—if you are selling anything, you must also have a CRS# Electricity: If you use a generator, it must have a muffler attached. Insurance: Food vendors must submit proof of a current one million dollar liability policy with their application. Carnival and simulator ride providers must provide proof of a current three million dollar liability policy* with their application. Application Checklist - Did you include the following? Return this completed application to the Downtown Association. Keep a copy for your records! Waiver and Conditions The undersigned vendor releases and holds harmless the City of Farmington, the Farmington Downtown Association, and the property owners and/or tenants; and waives all rights, without limit upon or liability for use of their property and facilities. The undersigned agrees that the Farmington Downtown Association may use any photos submitted or taken for publicizing the event. The undersigned further agrees that failure to abide by the rules stated in this application can lead to immediate expulsion with no reimbursement or legal recourse whatsoever. I have read these rules and agree to the terms of this contract. MUST BE SIGNED Signature Date REFUNDS Cancellation deadline is July 11, 2014.
