GUIDELINES FOR COMPLETING THE AFFIDAVIT The attached affidavit requires a detailed explanation in your own words, of exactly what happened during this incident. Your statement needs to include specific facts about WHO, WHAT, WHERE, WHEN, WHY, AND HOW. Accurately describe events in order of occurrence. Specify, if applicable, what type of weapon was used, right or left hand, open or closed fist, color of clothing or vehicle, what was stolen, resulting damage, what were the exact statements made by the suspect, etc. Try to avoid general terms like Ahead@ and Abody@. Tell exactly where physical injuries were sustained, i.e. right cheek, stomach, left rib cage, upper right thigh, etc. The affidavit must be in your own words.
