7/30/2010 DOUGLAS COUNTY SAFETY PROGRAM Near-Miss/Incident Reporting Program Purpose The purpose of this program is to track incidents which have occurred or potentially could occur causing bodily injury or property damage. The Risk Manager shall study these reports to determine methods to eliminate these hazards, their potential injury, and property damage. This program is not intended as a substitute for First Reports of Injury reports but as an addition to them. Incidents resulting in injury must still be reported on a First Report of Injury form. Application All County work sites Guidelines Identifying and Reporting Near-Miss Incidents 1. Any employee having knowledge of an incident or hazardous situation in which injury could have occurred or property damage did occur and the incident involves any type of hazard should immediately report it to their Supervisor using the Near-Miss Incident Report Form. 2. The Supervisor must immediately complete or ensure completion of the reporting form and take action to eliminate or mitigate any identified hazards. Any action taken must be documented on the attached form. 3. The Supervisor will forward copies to the Department Manager, safety representative, Human Resources, and Risk Manager. 4. Examples of reportable situations include a vehicle striking or narrowly missing a large animal; a lifting chain breaking and dropping an object; boxes stored too high on a shelf; etc. Corrective Actions 1. If an immediate solution is available to correct the cause of the incident, the Supervisor should immediately implement the solution to eliminate the hazard. 2. If an immediate solution is not available to correct the cause of the incident, any short-term action (such as warning signs, barriers, taking equipment out of service, etc.) should be immediately implemented. Long-term corrections should be initiated and reviewed with appropriate decision makers. When the solution is determined to be feasible, corrective action should commence at the earliest possible time. Follow-Up 1. When an employee experiences a near-miss incident and reports it to the Supervisor, the employee should ensure that action is taken. Whenever possible, to reduce or eliminate the hazard, the Supervisor should inform the employee of any actions taken. 2.
