Show graphic scale and north arrow. 2. Provide vicinity map showing site’s relation to surrounding area, including designation of specific phase, if necessary. 3. Provide existing and proposed contours. 4. Delineate all state waters located on or within 200 feet of the project site. 5. Delineate 25-foot undisturbed buffers of state waters and 50-foot buffers along designated trout streams from wrested point of vegetation. 6. Identify the project receiving waters and describe all sensitive adjacent areas – such as streams, lakes, drainage ditches, residential areas etc., which might be affected. 7. Variance from GA. E.P.D. required for encroachment in 25 or 50 foot state waters buffers. 8. Show double row Type –C silt fence between land disturbing activity and state waters, and wetlands. (No Exceptions) 9. File notice of intent and notice of termination with GA. E.P.D. and Dawson Co., if land disturbance is one acre or more or within 200 feet of state waters. Submission must be 14 days prior to start of land disturbance activities. ---PAGE BREAK--- 2 10. Phase E&SC plans into an initial perimeter control E&SC plan, intermediate E&SC plan for grading and drainage, and a final phase E&SC plan. 11. Show total and disturbed acreage (the disturbed area shall be the total estimated disturbed area of the primary and secondary permittees) of the project or phase under construction. 12. Provide calculations for required NPDES fee. Fees are $40.00 per disturbed acre paid to Dawson Co. and $40.00 per disturbed acre paid to E.P.D. (Show on cover and E&SC sheets). 13. Show soil series for project site and their delineation. 14. Show limits of disturbance on E&SC plans. 15. Provide revision and/or initial date on E&SC plans. 16. Provide description of existing land use at project site and description of proposed project. Include land lot and district numbers for site location. Describe critical areas and what measures will be utilized for these areas. 17. Provide name, address, e-mail, and phone number of developer/owner. 18. Provide name and phone number of 24 – hour local erosion, pollution and sediment control contact. 19. Show Level 2 certification number, signature and seal of qualified plan designer. 20. Provide an E&SC plan for a typical lot and each situational lot. 21. Provide BMP methods of containment and disposal procedures for concrete truck or mixer wash out. Washout of chutes on site is prohibited. 22. Provide a narrative for storage location, method of containment and emergency procedures in the event of a spill or reportable quantity of petroleum products. 23. Provide a description of practices to provide cover for building materials and/or other chemicals with respect to storage, clean-up and disposal. 24. Provide construction activity schedule – show anticipated starting and completion dates, and detailed sequence of events for all activities, including but not limited to: • Installation of sediment control measures • Installation of temporary sediment basins • Installation of detention facilities • Clearing, Grubbing and grading operations • Grassing – including mulching, temporary and permanent vegetation • Maintenance of erosion and sediment control measures ---PAGE BREAK--- 3 • Installation of Water Quality devices • Final landscaping grassing, cleaning of storm drains, etc. 25. Provide vegetative plan for all temporary and permanent vegetative practices, including species, planting dates, seeding, fertilizer, and mulching rates. 26. Show location and detail of erosion and sediment control practices, using uniform coding symbols from the manual for Erosion and Sediment Control in Georgia, Chapter six. Practices may include, but not limited to: • Construction exit • Sediment Barriers • Retrofitting • Storm Outlet Protection • Temporary sediment basin and calculations • Storm drain inlet sediment traps • Channel stabilization and vegetation • Check dams • Rock filter dams • Down drains • Temporary creek crossings • Mat blankets • Other 27. Show location of topsoil stockpile on plan. 28. Provide location, details and calculations for Water Quality devices. 29. Provide a minimum of 67 cubic yards per acre of sediment storage. 30. Show storm drain pipe and weir velocities and provide appropriate outlet protection to accommodate discharges without erosion. MUST BE INCLUDED IN NOTES ON PLAN Section 2: ---PAGE BREAK--- 4 1. The escape of sediment from the site shall be prevented by the installation of erosion and sediment control measures and practices prior to, or concurrent with, land disturbing activities. 2. Erosion control measures will be maintained at all times. If full implementation of the approved plans does not provide for effective erosion control, additional erosion and sediment control measures shall be implemented to control or treat the sediment source. 3. Additional erosion and sediment control measures and practices will be installed if deemed necessary by the on-site inspector. 4. Any disturbed area left exposed for a period greater than 14 days shall be stabilized with mulch or temporary seeding. 5. Erosion and sediment control measures and practices to be inspected daily. 6. Cut and fill slopes shall not exceed 3H: 1V on residential projects and lots, and lots shall not exceed 2H: 1V on all other projects. 7. Weekly erosion and sediment control reports shall be submitted to the Development Department starting with the issuance of the development permit and ending when the project is released by the inspector. 8. “ I certify that the permittee’s erosion, sedimentation and pollution control plan provides for an appropriate and comprehensive system of best management practices required by the Georgia Water Quality Control Act and the document ‘Manual for Erosion and Sediment control in Georgia’, published by the State Soil and Water Conservation Commission as of January of the year in which the land disturbing activity was permitted, provides for the sampling of the receiving water(s) or the sampling of the storm water outfalls and the designed system of Best Management Practices and sampling methods is expected to meet the requirements contained in the General NPDES Permit NO.
