CORTLAND COUNTY HEALTH DEPARTMENT DIVISION OF ENVIRONMENTAL HEALTH LEAK DETECTION SYSTEM TESTING RECORD FOR 200_ (Log date of test/inspection) JAN FEB MAR APR MAY JUNE JULY AUG SEPT OCT NOV DEC Note: Petroleum Bulk Storage Regulation Part 613.5 requires that the owner or operator monitor for traces of petroleum at least once a week. This would includes running leak test on ATG (save print out) and/or running test on interstitial monitors (save print out) or visual check of interstitial space in tank. All monitoring systems must be inspected This would include such things as sounding of panel alarm and/or checking the sensor/ probes status (keep printout) etc. If your system is using various types of sensors/probes, the system should be maintained according to the manufacturer’s recommendation.
