CSB Meeting September 27, 2016 COMMUNITY SERVICES BOARD OF CORTLAND COUNTY Meeting Minutes – September 27, 2016 4:00 P.M. ROOM 304 – COUNTY OFFICE BUILDING Members Present: Lisa Cutia (Chair), Aaron Baier, Donald Noble, Elizabeth Haskins, Jeff Beal, Lisa Hoeschele, Marie Walsh, Michael Johnson, ReBecca Smith and Tadeus Kowalski Members Excused: Christopher Driscoll, Christopher Hyde, Jackie Leaf, Reynolds and Knolls Others Present: Frances Burnham (MC ARC), Alexandra Huntington-Ofner and Gail Spitzer (Secretary) Call to Order: By Chair, Lisa Cutia at 4:01 p.m. Agenda Item: Welcome new member – Elizabeth Haskins Agenda Item: Approval of Minutes of August 23, 2016 Chair, Lisa Cutia, asked for a motion to approve minutes  Motion made by Aaron Baier and seconded by ReBecca Smith – no further discussion, minutes unanimously approved Introductions Message from Mark Thayer: Mark is currently attending the Annual State meeting of the Conference of Local Mental Hygiene Directors Gail Spitzer will relay some information on his behalf. Mark sends his apologies for not being able to attend CSB today Mark is not be able to attend the MH Subcommittee meeting on Monday as he will be meeting with the Office of Medicaid Inspector General (OMIG) in the informal first meeting – the formal, legal hearing has been moved from October 6 to early December Wednesday, Regional Planning Consortium (RPC) stakeholders’ meeting; currently no representatives registered from Cortland County (same day/time as CD Subcommittee, Mark requesting postponing that meeting) encouraging people to attend RPC: Holiday Inn, Liverpool, Wednesday, October 5, 10:00 a.m. to 12:00 noon – Reminder that the State mandated County Annual Report data collection form will be sent in October to all agencies – submission in early December Gail reported that the County provided training last week in preparation for the new website – training circumstances were not optimal – target mid- October to “go live” – meeting minutes will, once again, be posted in the near future Lisa Hoeschele asked Alexandra Huntington-Ofner if there was any new contact information for Trauma Response Team since Jennifer O’Callaghan was no longer point person – Alex: Seven Valley Health Coalition (SVHC) has provided a cell phone which will be the contact number; will be carried by the lead of on call team member – SVHC also writing grant to support some staff time – all new contact information will be updated and shared Request for information from Mark: Marie Walsh – Riverview Apartments Housing Development nearing completion – 39 unit apartment dwelling, 20 supported by NYS Office of Mental Health (SPSRO) supported model – anticipated date of completion December 2016-January 2017 – application available on Catholic Charities website: www.ccocc.org or through partner/co-owner Christopher Communities, Inc. / for supportive housing service type of residence, contact Shari Weiss – approximately 40 applications submitted to date; top priority given to people coming out of State hospitals or higher levels of care / most apartments are one bedroom, three apartments are two bedroom / rent extremely reasonable; HCR (Home and Community Renewals) – for MH community, the subsidy is built in; no more than 30% of person’s income / staff on sight (State requesting 24/7); peer and case managers / more information, contact Marie, Mike Pisa or Shari Weiss / Application, FAQ and schematic posted on CCOCC website / One unit adapted for blindness and one for deaf, fully adapted apartments increased to 6 – elevator, wide halls Dr. Kowalski – Update regarding coverage at Cortland Regional Medical Center (Lisa Cutia thanked him for serving as a member of CSB and participating in process – will be missed) Finalizing arrangements for new short term will utilize locum tenens Marie Walsh – Update Children’s Health Home (HH) transition – currently on track to take place in December (intensive case management for children with serious mental illness will become a HH model similar to Adult HH – population has expanded to include youth with complex health care needs and eligibility based on diagnosis has expanded to include a history of trauma – staff will not be limited to caseloads of 12: regulations changing to provide service based on need, rather than model – there is no longer a requirement to discharge by a certain time regardless of need / difference between Adult system has one HH – Children’s model will have two HH; one CHHUNY (Children’s Health Homes of Upstate NY) and the other Catholic Charities of Broome County / Children in foster care automatically qualify for HH / first step to transition children already in programs and any on waiting lists, building capacity (staff, etc.) / referral process still a unclear – as details and processes are set, Marie will keep everyone updated / Dr. Kowalski asked Marie to include the hospital staff when providing updates ---PAGE BREAK--- CSB Meeting – September 27, 2016 Agenda Item: Discussion – Provider updates Marie Walsh – Catholic Charities: Transitional Housing – 10 units on Charles St. referrals are run through DSS – diagnosis not required – this is an independent project; staffed by CCOCC helping temporary residents move forward, 2 units can accommodate families / Shari Weiss contact person Lisa Hoeschele – Family Counseling Services: Offices may be moving to larger facility – currently in five different locations – this would allow consolidation and the addition of primary care / OASAS and OMH have approved, waiting for DOH Mike Johnson – Wishing Wellness Center: Referral sources have changed to include Probation, Adult and Child Protective Services (DSS), three calls recently from the Hospital (CRMC) and an increase from within the community as word spreads about Center Lisa Cutia – Probation: Very pleased at the increase in number of options available; programs and services, within last five years, i.e. drug programs and housing options – extremely helpful / Probation Department increasing drug screening from standard to “trace test” – much more comprehensive – also helpful in determining if someone is not taking a medication they should be Alexandra Huntington-Ofner – Pumpkin Festival and Halloween Parade; good opportunity to share information Jeff Beal – DD Subcommittee: Next newspaper article is in process, help families understand services available and how to connect / speaker at last meeting from OPWDD to explain Self Directed Services; individuals and families receive and manage funds from State with the help of a broker (see DD Subcommittee minutes from October 4, 2016) can greatly improve quality of life for many with disabilities Aaron Baier – Access to Independence: October is National Disability Employment Month – many events planned, first working with 4 or 5 school districts, October 6 / Business Leader Breakfast October 13 – businesses and Chamber of Commerce – panel discussion: incentives and benefits panel / October 20, Workshop panel overcoming mental health stigma / October 27 workshop panel to discuss employment; to include peer success stories / self-advocacy and self-determination / October 20 Cortland Works Career Center hosting employment fair at JM McDonald Sports Center / New regulations from State coming regarding Access VR Fran Burnham – MC ARC: OPWDD is changing definition of priorities relative to housing; easing the process / submitted request for “letter of support” to Mark Thayer – help secure small grant to be used for recreation programs Camp ARCO Elizabeth Haskins – Office for the Aging: Increase in number caring for family members with developmental disabilities, asking for advice / State received significant money to expand and enhance “NY Connects” program (No-Wrong-Door) under Affordable Care Act – for anyone in need of long term support – person centered, options counseling / Medicare enrollment period – people can receive information about plan changes, etc. / New legal team at the State Office for Aging now mandate consent forms and processes similar to current HIPAA regulations – their information is shared with State via “Health Commerce” data base – significant impact on office; some refuse to sign or give consent – funding is at risk Next Meeting: October 25, 2016: CSB 4:00 & New member orientation 3:00 both in Room 302 – Next Joint meeting December 13, 4:00 in Legislative Chambers (Pre-CSB – Tuesday October 18 – 12:30 p.m. @ MH Clinic and change date of CD Subcommittee to Wednesday, October 12) Agenda Item: Adjourn Chair, Lisa Cutia, adjourned the meeting and thanked Dr. Kowalski for his time with the Board – meeting adjourned at 5:02 p.m. Minutes submitted by Gail L.
