Supervising Early Intervention Services Coordinator Page 1 I 2 Jurisdiction: Cortland County Jurisdictional Class: Competitive Adopted: 5/15/09 Amended: 4/22/10 SUPERVISING EARLY INTERVENTION SERVICES COORDINATOR DISTINGUISHING FEATURES OF THE CLASS: The work involves responsibility for overseeing and coordinating intervention services for children with developmental delays/disabilities and their families. The incumbent receives referrals, develops and implements the service plan, coordinates ongoing services and provides community outreach and public information services. Work is performed under the direct supervision of a higher level administrator with leeway allowed for exercising independent judgement in carrying out the duties of the work. Does related work as required. TYPICAL WORK ACTIVITIES: (Illustrative only) Assigns, directs, reviews and evaluates the work of subordinate staff; Plans, develops, implements and evaluates Early Intervention program services to ensure compliance with NYS regulations; Assumes all regulatory responsibilities as the Early Intervention Official Designee; Ensures program compliance with all grant work plan requirements; Meets with and interviews or coordinates appointments with individuals and/or families to explain Early Intervention regulations, parent rights, and program options and gathers information; Meets with families and service providers to develop and coordinate individualized family care plans; Meets with representatives from public and private agencies to coordinate services for mutual clients, and make and receive referrals; Contacts evaluation team and attends evaluation; Maintains ongoing contact with children and their families including making home visits and phone calls to review services being provided and make changes or referrals as needed; Meets with children and family prior to discharge to facilitate their transition into ongoing care or the community; Develops educational workshops and public information regarding early intervention services and meets with physicians and public and private community agencies to explain services; Gathers statistical data for program reports as required; Creates and maintains a variety of records and documents related to early intervention services such as medical records, progress notes and service plans by utilizing a personal computer. FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Good knowledge of early childhood development; Good knowledge of Federal and State early intervention program and eligibility requirements; Good knowledge of public and private agencies providing early intervention services to children and their families; Ability to communicate effectively both orally and in writing; ---PAGE BREAK--- Supervising Early Intervention Services Coordinator Page 2 I 2 Ability to establish and maintain effective working relationships with others; Ability to coordinate and develop service plans; Ability to make appropriate referrals; Ability to communicate effectively both orally and in writing; Ability to understand and interpret complex oral instructions and/or written directions; Ability to plan and schedule workload to maximize capacity; Physical condition commensurate with the demands of the job. MINIMUM QUALIFICATIONS: Either: Master’s degree in social work, counseling, nursing, sociology, or other closely related health services field, AND three years of experience working with young children and their families in a service coordination/case management role; OR Bachelor’s degree in nursing, social work, counseling, sociology, or other closely related health services field, AND five years of experience working with young children and their families in a service coordination/case management role. SPECIAL REQUIREMENTS: 1. Participation in introductory service coordination training pursuant to Section 69-4.4 of Article 25 of the New York State Public Health Law. 2. Possession of a valid New York State driver’s license is required. Failure to maintain a NYS driver’s license including driving privileges throughout employment will result in disciplinary action or dismissal.
