INDIVIDUAL SEWAGE DISPOSAL SYSTEM INSTRUCTION SHEET 1) Review and fill out the area in the APPLICATION FOR SEWAGE DISPOSAL PERMIT within the box. Before this office will conduct a site and soil appraisal, and before you may legally initiate any construction, the application(s) must be returned to this office. 2) If a new well is proposed, fill out the APPLICATION FOR WATER WELL CONSTRUCTION. 3) Three percolation (perk) test holes and a deep hole will be required in order to design the system. All excavations must be a minimum of 100 feet from any existing or proposed subsurface water supply and a minimum of 100 feet from a water source or body of water. Locate the excavations roughly as shown below. When the holes are dug, contact this office for an appointment for the test. a) Perk test. Three cylindrical excavations approximately 30 inches deep and 12 inches in diameter are required. b) Deep hole. Hole depth is required to be 9 feet in sandy gravelly soil, and 5 feet in all other soil types (or to continuous rock). 4) When the perk test scheduled time is known, pre-soak the holes by filling each hole the evening before the scheduled perk test. If the holes are not empty the next morning, the soil may not be suitable. About 15 gallons of water should be left next to each perk hole for the test. 5) This Department will then conduct a site and soil appraisal. The application fee must be paid before a representative of this Department will inform you of the results. If the site is satisfactory for development, this office will issue a CONSTRUCTION PERMIT for the sewage disposal system. The construction permit will include specifications which your contractor must utilize in the construction of the sewage disposal system. 6) When the sewage disposal system is complete, notify this office for a final inspection. THIS DEPARTMENT MUST EXAMINE THE SYSTEM BEFORE IT IS COVERED WITH SOIL. 7) A CERTIFICATION OF COMPLETION will be issued when a representative from this Department determines upon inspection that the sewage disposal system conforms to the specifications stated on the CONSTRUCTION PERMIT.
