Cortland County Coordinated Children Services Initiative 60 Central Ave. Cortland, New York 13045 Phone: [PHONE REDACTED] e-mail: [EMAIL REDACTED] Chris Driscoll, Coordinator Alma Johnson, Parent Partner Cortland County CCSI Eligibility Criteria For a child or youth to be eligible for CCSI, the following conditions must be met: 1) The child or youth lives in Cortland County 2) The child or youth is school age 5-21 3) The child or youth is at risk of residential placement, out of home placement or inpatient treatment (as defined below) 4) The child or youth has received services from three or more agencies A child or youth is at risk of residential placement if any one of these conditions is met: There is a current mental health evaluation recommending residential placement Committee on Special Education (CSE) has approved/is considering residential placement There is a pending application for a Residential Treatment Facility There is a recommendation by the Dept. of Social Services for placement There is a recommendation by Probation for placement OR A child or youth may be at risk of residential placement if any three of the following are present: Multiple failed mental health outpatient attempts Multiple probation reports History of hospitalizations Previous failed foster care placement(s) Multiple out-of-home placements (including drug & alcohol placement) Diagnosis of emotional or behavioral problems Designation of severely emotionally disturbed History of abuse/neglect in family Child or youth has experienced a previous residential placement Multiple failed school placements School problems, at risk for dropping out CSE involvement Juvenile Delinquency (JD)/Person In Need of Supervision (PINS) designation Substance/alcohol abuse Failed adoption A sibling has had a residential placement Previous child protective referrals Previous legal history - Juvenile Delinquency (JD)/Person In Need of Supervision (PINS) History of parental substance/alcohol abuse Multi-stressed home environment Parent or Guardian has a severe developmental disability which has a direct impact on the youth Other:
