Paul Stock Aquatic and Recreation Center • Cody Parks, Recreation and Public Facilities • City of Cody, Wyoming 1402 Heart Mountain Street • Cody, Wyoming 82414 • (307) 587-0400 Corporate Membership Plans (CMPs) are available to qualifying organizations interested in purchasing membership passes for any of their employees and their families. Qualifications: • To qualify for a CMP, your organization must have and provide a valid federal taxpayer identification number. Non-profit organizations may instead provide a letter of verification or determination from the I.R.S. of their 501(3)(c) status. • Memberships may only be issued to current employees and their families. • Employees may either get an individual membership or a family membership, which includes passes for themselves, their spouse, and any children living in their household who are claimed as dependents as per I.R.S. guidelines. Payments: • Every membership within the organization has the same renewal date. • All employees’ membership fees are paid by the organization in one annual payment; employees do not pay the Recreation Center directly. • Payments may be made by check or credit card. • Electronic Funds Transfer agreements payments) are not available for CMPs. • New/additional employees who join the organization after a CMP has been initiated are eligible before the organization’s renewal date. We prorate the fees based on the number of full months remaining. • Memberships under a CMP may be transferred to another employee for a $10 administrative fee. The organization also pays the difference of the cost of membership type if the new pass type is of greater value transferring from an individual to family membership). This difference is prorated based on the number of full months remaining until the organization’s renewal date. There are no refunds for transferring to a pass type of lesser value. • All membership fees are non-refundable and must be paid in advance. Also, I hereby pledge to provide positive support, care and encouragement for my fellow competitors and league officials by following this Code of Ethics. I will encourage good sportsmanship by demonstrating positive support for all players, coaches and officials at every game. I will treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability. I understand that I am participating in a recreation activity and agree not to use profane language or engage in ‘trash talking’ at any time. I will lead, by example, in demonstrating fair play and sportsmanship to all my fellow competitors. I understand that the officials are there for my benefit and will agree not to direct any profane or threatening actions toward them (either verbal or physical). I understand that any violation of this code shall be cause for the Shoshone Recreation Board & Cody Parks & Recreation, in executive session, to deal with any behavior and/or conduct that is considered by the Board to be disruptive, destructive, and/or illegal. Behavioral problems may result in expulsion from the program without reimbursements. The Shoshone Recreation District/Cody Parks & Recreation Department does not provide any medical insurance or personal liability to cover participants during an activity. This is the responsibility of each individual involved in any recreation activity. NOTICE: The City of Cody reserves the right to take and use photographs of individuals using the Cody Recreation Center and/or participating in programs sponsored by the City of Cody. Such photographs are the property of the City of Cody, and may be used in brochures, advertisements and other promotional materials. All memberships are non-transferable.
