Enclosed you will find your review packet. Please read the following instructions carefully before completing the enclosed forms. You need to completely fill out all of these forms and return them to Clay County Social Services. When these forms have been processed, we will be making our unannounced visit to your home. YOUR PRIVACY RIGHTS: Read and sign the back page. Return with review packet. LICENSING REVIEW: Complete all questions, sign and date the form. OTHER INFORMATION: We will also need to see the following forms during our visit to your home: ➢ Training Record Log ➢ Fire and Storm Drill Log ➢ Record of Liability Insurance (If you have insurance, the County Attorney has requested that a copy of the cover page be placed in your file.
