Your Identity Theft Report number is If you believe that you are a victim of identity theft, there are a number of important steps for you to follow. Be prepared to document all unauthorized transactions and to be patient—in some cases the process can take a number of months. The first step in the process is to contact the Midland Police Department. A uniformed officer will meet with you and supply you with an Identity Theft Report number and any necessary forms to assist you in investigating your complaint. If you have Internet access, the officer will direct you to the appropriate forms to fill out online. Following this initial meeting with the police department, complete the necessary forms indicated by the police officer, making sure to document everything you do.
