(REV-08/2017) Application for Disconnection of Water Service Please advise the Village of Fair Haven which water service you prefer. After you sign this, return it to the Village Office. Upon approval, a copy will be mailed to you. DISCONNECTION – Service will be disconnected and billing stopped. When service is restored, there will be a charge of $175.00, new service charge. OR PAPER DISCONNECT – Service will be discontinued for twelve months or more and billing will be at a rate of 1/3 of the original billing. When service is restored, there will be an additional service charge of $35.00. There will be no reduction in billing on discontinued service of LESS THAN ONE YEAR. Water Approved Approval This notice will be reviewed annually and rates are subject to change with written notice to the water customers. Village of Fair Haven P.O.
