NEW YORK STATE DEPARTMENT OF HEALTH DEPARTMENT OF ENVIRONMENTAL CONSERVATION APPLICATION FOR APPROVAL OF SANITARY FACILITIES FOR REALTY SUBDIVISIONS NOTE: (Law requires that no subdivision or portion thereof shall be sold, leased or rented or any permanent building erected thereon until plans are approved by State Department of Health or Department of Environmental Conservation.) Application is hereby made for the approval of plans for realty subdivision as required by the provision of Title 11 of Article 11 of the Public Health Law, and Title 15 of Article 17 of the Environmental Conservation Law.
