City of Burley [PHONE REDACTED] Procedure to Connect, Disconnect or Transfer Power/Utilities FIRST: CALL CITY HALL ([PHONE REDACTED]) (INFORMATION WE WILL COLLECT OVER THE PHONE) 1. Mailing Address 2. Contact Phone Number 3. Primary and Secondary Names of owner/renter 4. Driver’s License Number 5. Date of Birth 6. Former Residence Address 7. Place of Employment (Primary & Secondary) 8. Two References – Name, Address and Phone Number 9. Establish Amount of Deposit and if Required SECOND: WHAT YOU WILL BRING TO CITY HALL 1. IF YOU ARE BUYING A HOME , please provide a copy of the Warranty Deed from the title company showing the purchasers name and address of property being purchased. IF YOU ARE RENTING please bring in a copy of your lease, or a City of Burley Landlord Form filled out by the landlord showing the renter’s name and address of the rental property. 2. Your current photo ID. 3. Payment of Deposit as discussed on phone call. * Deposit – Twelve Months (from this day back) history from your prior utility provider. Deposit amount is based on the location. * If transferring services from one residence within City limits to another, a deposit on one account can be transferred to the new residence. If the original deposit is less than the new deposit the difference must be paid, before services are connected.
