August 10, 2022 Seefried and Hepworth Lane Project Addendum #2 Item #1 – In the bid advertisement it was inadvertently published that the contractor would need to coordinate road work with another contractor. All road work will be completed by the contractor that gets the bid for the sewer line install. Contractor must verify sub grade material, install pit run, road base and asphalt. Item #2 – Contractor will be responsible for their own staking and surveying. Connect engineering will supply contractor with a CAD file of all currently completed survey data. Item #3 – All driveway approaches and intersections must be saw cut in a straight line. Paving should start at the start of the radius on the connecting streets. Item #4 – The road work on Hepworth and the sewer work on Seefried may be worked on at the same time provided at least one lane of traffic remains open for residential traffic. At least one road must be accessible. Item #5–A completed traffic plan must be submitted to Scott Murphy @ [EMAIL REDACTED] and Bingham County @ [EMAIL REDACTED]. Both Scott and Bingham County must approve the traffic plan prior to beginning any road closures. Item #6 –The contractor will be responsible for communicating with the residents along Seefried and Hepworth Lane about the progress of the work, road closures, interruptions of service. Additionally, the contractor must notify emergency services, Blackfoot District Transportation services, U.S. Postal Service, and Blackfoot Sanitation Department of road closures. Share the approved traffic plan with them. ---PAGE BREAK--- Item #7 – The following items for the sewer install will be supplied by the city. 1. Sewer manholes 3 ea with riser rings and covers a. Attached to the back of this document is the submittals for the sewer manholes 2. Water manhole riser rings and covers. a. Also below is a list of items that will need to be brought up to grade, and collars installed on the newly installed water line after pavement is placed. All of these items are installed and sit just below the 3/4 gravel. b. 7- inline water valve boxes c. 5- Hydrant valve boxes d. 2- 1030 frost proof ring and covers ( on the canal crossing water valve manholes ) Item #8 – At this point the County and the City will be requiring separate bid bonds. The county bond amount is 110% of the Engineers estimated cost for the entire project. The city Bond amount is 85% of the bid amount that is within city limits, which is approximately 1/3 of the total project. A permit from the county is required, a copy to be supplied to the city.
