Local Small Business Opportunity Program SUPPORTING DOCUMENTS APPLICATION PACKET CHECKLIST Submit copies of the following documents along with the application. All copies must be legible and true/complete copies of the originals. Failure to submit the required supporting documents with the affidavit shall result in the return of the registration application. ***If an item is not applicable to your business, please insert “N/A” in the space provided. SECTION I: Completed Local Small Business Opportunity Program (LSBOP) Registration Application for each person who has an ownership interest in the business. Check the type of business structure of your company. Provide the date that the business was started or was purchased by the applicant owner(s) listed in Section I above. If you operate your business out of your home, please check “YES”. Indicate the number of full-time and part-time employees and enter the total. List all NAICS Codes that apply to your firm. Provide the name of your bonding agent and the firm’s current bonding capacity. (If applicable) Write a brief description of your firm’s primary product, service, or work area. Then add three supporting descriptives which clarify and identify your specific product, service, or area of work. This is how your firm will be described in the Registration Directory should you be granted registration. Check the type of business activity which best describes your services. SECTION II: Signed copies of Corporate Federal Tax Returns including all schedules for the previous three years (for sole proprietors: individual tax returns) a. If firm has been in business less than three years, copies of Corporate Federal Returns for applicable years b. If firm is a Sole Proprietor and has been in business less than years, copies of individual returns for applicable years SECTION III: Indicate if the ownership interest of any of the owners has changed since the business was started or was purchased. If a change has occurred, tell us when (date) the change occurred. List the names of current members of the Board. Please show the ownership percentage owned by the individual(s), and/or the position held in the company. List the personal net worth of all owners and their ownership percentage in the business. SECTION IV: List three projects the company has worked on or completed. Include project owner, contact person’s name, address and telephone number, type of work performed, and dollar amount of project.
