APPLICATION INSTRUCTIONS HOME Investment Partnerships Programs Application ---PAGE BREAK--- GENERAL INFORMATION Purpose and General Applicable Provisions Under this Application, the Augusta Housing and Community Development Department (AHCD) seeks applications and development proposals from non-profits and for-profit entities to undertake affordable housing development projects. These projects will involve the rehabilitation and/or construction of affordable rental housing or the rehabilitation or construction of owner-occupied housing, that meet requirements established under 24 CFR Part 92 (HOME Investment Partnerships [HOME]) Program This application is to be used if you were funded for HOME funds in the initial application process and now you are seeking additional funds for another project. All project proposals must be located within the geographic boundaries of Augusta-Richmond County, Georgia. Application Review Process AHCD staff will perform a preliminary review of each application to determine whether it meets minimum submission requirements. The review will include a determination that each application is complete with all required documents and exhibits and consistent with applicable program requirements. AHCD will score and rank applications. (An application receiving a score below 70 will not be recommended for funding.) A recommendation will be given to the Director of AHCD. AHCD Director will conduct the final recommendation to be submitted to Administrative Services. Eligible Applicants Eligible applicants include 501(c)(3) or 501(c)(4) Community Housing Development Organizations, Non-Profit Corporations and Joint Venture Partnerships between one or more for-profits entities and one or more non- profit corporations for the purpose of responding to this application. Deadline for Application Submission All applications are to be submitted to the Augusta Housing & Community Development Department, located at 925 Laney Walker Blvd.., 2nd Floor, Augusta, Georgia, 30901. Acceptable methods for delivery are by certified mail, or other delivery-service requiring signature by an AHCD staff member. AHCD will not accept applications that are HAND DELIVERED. Application packets will be date and time stamped upon entry. APPLICATIONS RECEIVED AFTER THIS DEADLINE WILL BE RETURNED TO THE APPLICANT AND NOT CONSIDERED. 1. Cost of Proposal All costs associated with responding to this application are the sole responsibility of the applicant. This includes any costs associated with the hiring of a consultant or other services by the applicant to assist in the preparation of any component of the application. ---PAGE BREAK--- 2. Mis-Statements or Mis-Representations If it is determined that a recipient of the HOME Program funds has made a material mis-statement or mis-representation relating to the recipient’s application for or administration of a grant, Augusta will (at its discretion) require the recipient to repay the received funds to the City. APPLICATION SUBMISSION FORMAT All applications MUST be submitted in the following manner: 1. One original copy in a binder (signed in blue ink) and One complete loose copy must be provided. 2. The application should be assembled so that it follows a logical and consistent format. Cover pages are the preferred method of separating materials. Information should be of a consistent size (8.5 X 11). Fold-outs, brochures, photographs, etc. should also be formatted to these guidelines. 3. All applications are to include proof that the project has been discussed, presented and approved by the local neighborhood/community association. This activity is to be completed through the submission of correspondence on the official letterhead of the local neighborhood/community organization and marked as “Attachment # 1” 4. For each project type enclosed are forms which require completion and are a part of this application packet. ---PAGE BREAK--- HOME PROGRAM DESIGN Guiding Principles 1. APPLICANT CERTIFICATION Disclosure Statement: I/We, the undersigned, understand and agree that Augusta, Georgia will consider that a Conflict of Interest exists whenever any party to a transaction has a financial or family relationship or a professional or business affiliation with any other party to the transaction. The following list may not be all inclusive but is considered to be representative of some of the more typical Conflict of Interest: 1. The applicant or principals of the applicant are related to or have a business relationship with any Citizens Advisory Committee (CAC) member or their spouse. 2. The applicant or principals of the applicant are related to or have a business relationship with any Augusta, Georgia employee or employee’s spouse. 3. The applicant or principals of the applicant are related to or have a business relationship with any elected officials or their spouses. 4. The applicant or principals of the applicant related to the seller, seller’s spouse, or seller’s agent of any property to be financed with any proceeds which may result from this application. 5. The applicant or principals of the applicant, employees, agents, consultants, officers of the agency requesting funds has any financial interest in the project. 6. The applicant has any financial interest in the seller of the property or its agent. 7. The seller of the property or its agent has a financial interest in the applicant. 8. If this is a construction loan, the applicant has a financial interest in or is a director, employee or officer of the general contractor, architect, engineer, attorney, interim or participating lender, materials supplies, equipment lessors, or others pertaining to the construction of the subject property. 9. The applicant is aware of any other circumstances that may be a Conflict of Interest through the sale of the property or other matters. The Applicant is prepared and has the authority within its charter, by-laws, or through statutory regulations to enter into a contractual agreement with Augusta, Georgia for acceptance and use of HOME monies, and makes this application and certification with full cognizance of its governing body. 5. The undersigned hereby agrees and allows the release of any and all information submitted to Augusta, Georgia in regards to the representations made within this Application. Such information may include credit history and ratings verifications, confirmation of involvement in past development, and all other information as may be required by Augusta, Georgia to aid in making a determination as to the awarding of HOME funds to the Applicant. 6.
