CITY OF ANAHEIM REQUEST FOR PROPOSAL TO PROVIDE PROFESSIONAL SERVICES FACILITIES CONDITION ASSESSMENT GENERAL City of Anaheim, Public Works, Facility Services is requesting proposals from qualified consulting firms to conduct a facility condition assessment and inventory for eleven (11) of its buildings. The project seeks to assess the condition of City facilities and to prioritize capital improvement projects, repairs, and maintenance. Results of the study will assist Facility Services in managing resources and assets of the buildings as well as develop a monetary report to be used in the annual facility rental rate calculation. Specific information for each building is provided in Attachment A. Additional property information is available on the Orange County Office of the Assessor website. The primary deliverables of the study include a comprehensive inventory for each building and comprehensive condition assessment and lifecycle assessment of major systems including plumbing, mechanical, electrical, roofing, seismic risk, site access, parking and pavement, general code compliance, life safety compliance (e.g. ICC/NFPA), and Americans with Disabilities Act (ADA) compliance. The study will identify opportunities for replacing, repairing or upgrading various building components and systems using available sustainable and energy efficient technology. The facility condition assessment analysis study shall be composed of three separate parts: general facility condition assessment including a Condition Index (FCI) score (good (under 0.05), fair (0.05 to 0.10), and poor (over 0.10), detailed facility condition analysis, and preventative maintenance schedule. SCOPE I. Meet with City Representative(s) at beginning of task to determine schedule for on-site work at each location. Work directly with City assigned representative during all phases of on-site facility inspection. II. Conduct a comprehensive evaluation of City buildings, per Attachment A, documenting all system deficiencies, encompassing the structure, seismic risk, system, components of each including recommended ADA compliance items and opportunities for replacing, repairing or upgrading various building components and systems using available sustainable and energy efficient technology. Provide estimate of the cost and prepare a schedule to repair or renew elements as needed. The estimate is to be used for long term capital project planning. III. Assess system conditions and conduct a targeted evaluation of: 1. Exterior structure 2. HVAC 3. Electrical 4. Plumbing 5. Roofing ---PAGE BREAK--- 6. Elevators 7. Seismic Risk IV. Prepare a Facilities Condition Assessment Report of findings and facts for City Representative to review that summarizes the findings of Scope item I and II which shall include recommendations addressing facility deficiencies. The report shall include cost estimates for renovations, appendices of supporting data including photos of specific problems and a Facility Condition Index (FCI) score good (under 0.05), fair (0.05 to 0.10), and poor (over 0.10),. 1. Reporting and analysis shall be presented by site location, then by category and correction costs. 2. Deficiency information must include: i. Description of location ii. Priority 1. Priority 1- currently critical: require immediate action to: a. Return a facility to normal operation b. Stop accelerated deterioration c. Correct a cited safety hazard 2. Priority 2- potentially critical (year one): if not corrected expeditiously, will become critical within a year. Situations in this category include: a. Intermittent interruptions b. Rapid deterioration c. Potential safety hazards 3. Priority 3- necessary, not yet critical (year two-three): conditions requiring appropriate attention to preclude predictable deterioration of potential downtime and the associated damage or higher costs if deferred further. 4. Priority 4- recommended (year four-five): represent a sensible improvement to existing conditions. These items are not required for the most basic function of a facility; however, will either improve overall usability and/or reduce long term maintenance. 5. Priority 5- (year six-ten) iii. Type iv. Corrective information, including craft required to perform corrective action, quantity and timing data, and other details that will enable cost estimates, including labor and material, to be generated for budgeting purposes. 3. Recommendations for system components shall include the following: i. Frequency of preventative maintenance (PM) ii. Procedures associated with PM iii. Time associated with PM iv. Hazardous materials associated with each PM v. Set-up, wrench, and clean up time. ---PAGE BREAK--- REQUIREMENTS V. Provide clean, concise proposals that contain only information required responding to the needs of this project. Proposal must at minimum include the following: a. The proposal shall be received by the City’s Public Works, Facility Services Representative, Stanley Burks, no later than 5:00 p.m. on April 17, 2015 at 955 S Melrose St, Anaheim CA 92805. b. Proposed project schedule identifying key tasks and their associated duration. City desires to complete the Facilities Condition Assessment on or prior to June 30, 2015 c. General information describing firm size, office locations and relevant firm capabilities. d. Identify the team to be assigned to the project by name, including project manager and other key team members. Provide qualifications and resume for each team member. e. Submit copy of recent similar project that reflect the quality of their work. Provide information on recent projects similar in nature to the proposed project to document consultant’s expertise, experience and ability to complete proposed project in a timely manner. f. List of three project references with name, address, phone number and contact person(s). g. Identify in sufficient detail the consultants approach to and understanding of the project for each phase of work to include approach to quality control, quality assurance, methods for managing cost and management to ensure product delivery is on time and budget. h. Outline the projected hours to be spent on project by each team member for services described in the Scope to include professional service rate for each team member. i. A total, not to exceed, cost for requested services. COSTS VI.
