Melrose Anaheim, California 92803 TEL ([PHONE REDACTED]) FAX ([PHONE REDACTED] www.anaheim.net City of Anaheim DEPARTMENT OF PUBLIC WORKS Fleet & Facility Services February 25, 2015 RE: REQUEST FOR PROPOSAL TO PROVIDE FACILITIES CONDITION ASSESSMENT CONSULTANT SERVICES To Whom It May Concern: INTRODUCTION: The Fleet & Facility Services Division of the Public Works Department is requesting proposals for facilities condition assessment and inventory for Eleven (11) of its buildings. Results of the study will assist Facility Services in managing resources and assets of the buildings as well as develop long term capital needs. SERVICES TO BE PROVIDED: Proposed services are to be included as described in the attached Request for Proposal (RFP) and Attachment A. INFORMATION TO BE INCLUDED IN THE PROPOSAL: Proposal is to include at minimum all requirements listed in section V Requirements, items A thru I, as described in the attached Request for Proposal (RFP). SELECTION OF THE COMPANY: The proposal shall be received by the City’s Public Works, Facility Services Representative no later than 5:00 p.m. on April 17, 2015 at 955 S Melrose St, Anaheim CA 92805. A review Board selected by the Fleet & Facility Services Manager will review the proposals received. Upon conclusion of the review, and interviews, if required, the Review Board shall rank the consultant based on the following criteria: A. Ability of the consultants to perform the specific tasks outlined in the RFP within the desired time frame. B. Qualifications of the specific individuals who will work on the project. ---PAGE BREAK--- C. Reasonableness of the fees requested to do the work. D. Demonstrated record of success by the consultant on work previously performed for other municipalities. E. The specific method and techniques to be employed by the Consultant on the project. F. A “Quality Assurance Program” for services rendered. GENERAL INFORMATION: 1. All work shall be conducted when requested by the Fleet & Facility Services Manager or designated representative. Work shall be completed in a timely and expeditious manner. 2. This RFP does not commit the City to pay any cost incurred in the preparation of a response. 3. The City reserves the right to withdraw this RFP at any time for any reason. All firms preparing a proposal will be duly notified of any change in the status of this RFP. 4. The Consultant is expected to establish and maintain a close working relationship with the City staff throughout the duration of the contract. 5. The Consultants shall submit five copies for review in the preparation of this proposal and one electronic version. 6. The Consultant is expected to provide the City the necessary services as described in the attached RFP, within the established time frames.
