A typical situation involves machinery made of components that are individually listed (certified as safe for its intended use) but the final assembled machinery is not separately listed. The City’s Building Inspector will advise you when certain equipment is not adequately listed. You should then retain a qualified independent testing laboratory to prepare a certification report indicating the equipment is suitable for its intended use, as well as any limitations that may be relevant. The testing laboratory will inspect the equipment in the field and will prepare a report certifying the equipment is safe. After receiving the report, also known as a field listing report, the City Building Official can approve installation and use of the machinery. The certification report should include the following: 1. Address where the equipment will be installed and used. 2. The make, model, and serial number of the equipment and other information that identifies the particular equipment being evaluated. 3. A statement that the equipment is suitable for its intended use. 4. Any limitations on the use of the equipment. 5. For electrical installations, requirements of the equipment including the minimum and maximum breaker or fuse type and size. A complete list of qualified independent testing laboratories can be found online at http://www.osha.gov/dts/otpca/nrtl/index.html. Not all laboratories provide verification of compliance for every type of standard, so you should contact the facility you select to clarify the needed testing. Although the City does not recommend particular facilities, for your convenience we are providing information on facilities that are recognized by OSHA as having the necessary qualifications and that are experienced in Orange County projects: For Electrical Equipment: Canadian Standards Association Underwriters Laboratories, UL Irvine, California Santa Clara, California (949)733-4300 (408) 985-2400 Toll Free (800)463-6727 TUV Rhernland of North America, Inc Wyle Laboratories Carlsbad, California El Segundo, California (760) 929-1780 (310) 322-1763 ---PAGE BREAK--- 200 S.
