REQUEST FOR STATEMENT OF QUALIFICATIONS FOR PROFESSIONAL ARCHITECTURE, ENGINEERING AND PROJECT MANAGEMENT SERVICES ALPINE COUNTY BEHAVIORAL HEALTH SERVICES FACILITY Reply to: BRIAN PETERS DIRECTOR ALPINE COUNTY COMMUNITY DEVELOPMENT DEPARTMENT 50 DIAMOND VALLEY ROAD MARKLEEVILLE, CA 96120 [PHONE REDACTED] x425 [PHONE REDACTED] (fax) [EMAIL REDACTED] December, 2014 ---PAGE BREAK--- RFQ – AE Services Page 2 Alpine County, California, is seeking qualified architecture/engineering firms to provide professional consulting services associated with site selection, planning, design, project management and construction of the Alpine County Behavioral Health Services facility as outlined in the Alpine County Mental Health Services Act Plan 2014-2017. PROJECT DESCRIPTION This solicitation will select the Architect of Record (Architect) to be retained by Alpine County (Owner) under a Professional Services Agreement (PSA) for the services described above. Alpine County recently completed the development of the three year Mental Health Services Act Plan that identified future facility needs for Behavioral Health Services (BHS). The plan includes the development of a second “ w e l l n e c e n t e space and BHS administrative and counseling offices. The facility will be located in the Woodfords/Markleeville area. In FY 2010-11, Alpine County requested and received funds to establish a second wellness center. However, those plans were not implemented due to staff turnover. During the April 2014 community work session, the need for additional space was revisited and stakeholders agreed that the county did need more office and service space, but not necessarily a second wellness center. The Capital Facilities funds will be used to acquire a new building or space for MHSA administration and services. The focus of the building or space will be to offer MHSA services to children, transition age youth, families, adults, and older adults, providing: activity rooms for individual and group service delivery (including Play Therapy and additional wellness programs); and dedicated space for ACBHS administration. The total cost of all improvements including design fees, permits, site acquisition, on-site and off-site utilities, project management, construction, furniture, fixtures and equipment is e s t i m a t e d t o not exceed $3 million. The facility program is based on the Alpine County Mental Health Services Act Plan 2014-2017 conducted by Resource Development Associates, in collaboration with the Owner’s project stakeholders. The facility requirements, location, size, configuration, and significant attributes have not been defined. Specific project requirements and design elements shall be generated by the Architect and formalized in a Basis of Design Report, with the coordination and approval of the Owner. Specific elements of the design that need to be considered include: Location must serve the Markleeville area of Alpine County • Develop a design that provides full ADA-accessibility. Multiple entrances to accommodate multiple functions of the building. • Develop a design that incorporates a lobby, reception desk, secure file storage room, copy and workspace room, staff break room, kitchen, wellness center space, conference room, play therapy room, 5-6 counseling offices, 4-5 administration offices and adequate ---PAGE BREAK--- RFQ – AE Services Page 3 on-site parking • Develop a design that is in compliance with all applicable local, state and federal environmental regulations SCOPE OF SERVICES The scope of services includes, but is not limited to: Data gathering, field survey, planning, schematic design, site evaluation, design development, cost estimates, scheduling, phasing, client review, stakeholder coordination, construction documents, construction support services, construction administration, facility commissioning, and project close-out. The Owner reserves the right to negotiate additional services or change the project delivery method with the selected firm(s) as it may deem necessary to fulfill the project requirements. SCHEDULE Submit SOQ 6, 2015 Interviews (if required) 20-23, 2015 A/E 3, 2015 Notice to 17, 2015 Site Selection August 30, 2015 Design Completion & Bid 31, 2015* *This date for completion is dependent upon completion of site selection by August 30, 2015 SUBMITTAL CONTENT Format SOQs should be typed and submitted on 8.5-inch x 11-inch page portrait format, softbound, and shall include a cover with the name and address of the Consultant and marked “Alpine County Behavioral Health Facility Statement of Qualifications for Professional Architecture / Engineering Services”. SOQs should not exceed twenty-five (25) pages in length, and should not include unnecessary promotional material. SOQs may use both sides of a page, but each side will be counted as a page. The page limit does not include the outside cover, section dividers, cover letters, contract comments, or duplication of the organizational chart. All pages shall be sequentially numbered and each section should be separately and clearly tabbed. SOQs should include a complete table of contents for the entire SOQ with respective page numbers opposite each topic that is included. Letter of Transmittal The Letter of Transmittal shall be addressed to Brian Peters, Director, Alpine County Community Development Department, 50 Diamond Valley Road, Markleeville, CA 96120, and must, at a minimum, contain the following: a. Identification of the firm and/or individual that will have the authorization to commit to the contractual terms and conditions detailed herein. Identification shall include legal company name, corporate address, telephone, fax number and e-mail address. Include name, title, address, telephone number and e-mail address of the contact person during the SOQ evaluation phase. b. Identification of proposed sub consultants, including legal company name, contact person’s name, address, and phone number. Include a description of the working ---PAGE BREAK--- RFQ – AE Services Page 4 relationship between primary firm and individual and sub consultants, if applicable. c. Acknowledgment of receipt of all RFQ addenda, if any. d. A statement to the effect that the SOQ shall remain valid for a period of not less than 180 days from the date of submittal. e. Signature of a person authorized to bind the firm and/or individual to the terms of the SOQ. f. A statement to the effect that by signing the letter of transmittal, the firm and/or i ndividual is attesting that all information submitted with the SOQ is true and correct. Required Information This section of the SOQ should establish the ability of the firm and/or individual to satisfactorily perform the required work by evidence of experience and demonstrated competence in performing work as described in this RFQ. 1. Description of the team organizational structure, including a chart identifying the Project Manager; key personnel, including Designer, project Architect, Interior Designer, and Construction Administrator; sub consultants for Civil, Structural, Mechanical, Plumbing and El ectrical design; and responsibilities of team members. 2. Provide a general description of the firm’s financial condition and identify any conditions bankruptcy, pending litigation, planned office closures, staffing changes, impending merger or other) that may impede firm and/or individual’s ability to provide the requested services. 3. State of California Architecture License and conformance with California Administrative Code. 4. Information on Firm: • Provide brief description including: Name and main/home office location, year established, legal status, and ownership. • For firms with multiple offices, indicate location where work will be performed and briefly summarize years of operation, staffing. • Staffing – Provide the following: o Principal Architect with resume indicating years of experience, relevant recent experience, registration, and list of responsibilities. Include only work experience performed with current firm. o Project Manager with resume indicating years of experience, relevant recent experience, registration, and list of responsibilities. Include only work experience performed with current firm. o Other Key Personnel assigned to the project with resume indicating years of experience, relevant recent experience, registration, and list of responsibilities. Include only work experience performed with current firm. 5. Qualifications and experience in providing A/E services for similar public facilities, familiarity with law enforcement and similar government facilities. Provide a minimum of three project examples completed since 2010, with the names, addresses, phone numbers and email addresses of contact persons. 6. Ability to provide a local presence for client and job site activities, including prompt response during construction. 7. Experience with local (within Alpine County, Lake Tahoe Area and the Reno-Carson Valley) construction materials, methods, and costs. 8. Approach / Methodology: Describe your understanding of the project, significant issues to be addressed and your specific approach to the design process for this facility. 9. The Firm and/or individual shall submit evidence of ability to provide insurance in the amounts and coverage as required in the attached Model Professional Services Agreement (Exhibit A) SOQ Preparation Expenses The County shall not, in any event, be liable for any expenses incurred by any firm or individual in the preparation of its SOQ. Firm or individual shall not include any such expenses as part ofi ts SOQ. SOQ preparation expenses include the following: 1. Preparing its SOQ in response to this RFQ; 2. Submitting that SOQ to the County; 3. Negotiating with the County any matter related to this RFQ; or 4. Any other expenses incurred by a firm or individual prior to the date an agreement is executed. SELECTION PROCESS A selection committee from the Owner will review the Statements to determine which respondent is best qualified to perform the work. Depending upon the number and qualifications of respondents, the Owner may select directly from the Statements of Qualifications, or may develop a short list of firms for interviews and final selection. The decision as to the process, timing, and selection will be based entirely on the judgment of the Owner selection committee. NON DISCRIMINATION The consultant or its sub consultants shall not discriminate on the basis of race, color, national origin, or sex in the performance of this contract. Failure by the consultant to carry out these requirements is a material breach of contract, which may result in the termination of this contract or such other remedy, as the recipient deems appropriate. In the event your firm/team desires additional information, the Owner will endeavor to provide such information as expeditiously as possible; however, the Owner will not be responsible for any delay resulting in respondent’s inability to meet the deadline for submission of the Statement of Qualifications. In addition, the Owner will not be responsible for costs associated with preparation or submittal of the Statement of Qualifications. COUNTY RIGHTS ---PAGE BREAK--- RFQ – AE Services Page 6 The County may investigate the qualifications of any firm or individual under consideration, require confirmation of information furnished by a firm or individual, and require additional evidence of qualifications to perform the Scope of Work described in this RFQ. The County reserves all rights described below: 1. Accept or reject any and all SOQs, or any item or part thereof, or to waive any informalities or irregularities in SOQs; 2. Issue new or subsequent RFQs; 3. For any reasons, withdraw or cancel this RFQ, or any part thereof at any time without prior notice and the County makes no representations that any contract will be awarded to any firm or individual responding to this RFQ; 4. Require confirmation of information furnished by firm or individual or for the firm or individual to provide additional evidence of qualifications to perform the work or to obtain information from any source that has the potential to improve the understanding and evaluation of the SOQs; 5. All SOQs shall become the property of Alpine County and shall not be returned; 6. All SOQs submitted may become public records under the laws of the State of California and the public may be given access thereto after the formal process has been completed. Submitted SOQs are not to be copyrighted; 7. Postpone SOQ openings for its own convenience; 8. Remedy or overlook technical errors in the RFQ process; 9. Appoint an Evaluation Committee to review SOQs; 10. Seek the assistance of outside technical experts in SOQ evaluation; 11. Approve or disapprove the use of particular sub consultants; 12. Establish a short list of firms or individuals eligible for interview after review of written SOQs; 13. Negotiate with any, all or none of the firms or individuals responding to the RFQ; 14. Solicit best and final offers from all or some of the firms or individuals; 15. Award the contract as a whole, by category, or by any combination that best meets the need of the County. AWARD OF CONTRACT Upon selection of a successful proponent(s), a County standard form Professional Services Agreement will be provided, a copy of which is presented in Appendix A for proponents’ review. Proponents shall examine the contract and identify any request for modification from these standard terms as part of the proposal. INDEPENDENT CONTRACTOR The successful proponent(s)will work in the capacity of an independent contractor for all purposes under this contract, if awarded, and shall not be entitled to any or all rights, privileges, benefits and remuneration of either an officer or employee of Alpine County, unemployment insurance, retirement, paid holidays, vacation and sick leave, medical insurance plans, any other job benefits. Contractor will be responsible for payment of state and federal taxes resultant from contractual earnings. The contractor will work under the direction of the County Community Development Director or his designee. This Agreement is subject to the budget and fiscal provisions of applicable laws, the County Code and rules of the County. Unless otherwise stated in the Agreement, charges will accrue only after execution of the Agreement, and the amount of County's obligation hereunder shall not at any time exceed the amount stated in the Agreement for the purpose and period stated in the Agreement. This Agreement will terminate without penalty, liability or expense of any kind to County at the end of any fiscal year if funds are not appropriated for the next succeeding fiscal year. If funds are appropriated for a portion of the fiscal year, this Agreement will terminate, without penalty, liability or expense of any kind at the end of the term for which funds are appropriated. This Agreement will terminate if funds that were previously appropriated for this Agreement are reduced, eliminated, and/or re-allocated by County as a result of mid-year budget reductions. County has no obligation to make appropriations for this Agreement in lieu of appropriations for new or other agreements. County budget decisions are subject to the discretion of the County Administrative Officer and the Board of Supervisors. Contractor’s assumption of risk of possible non-appropriation is part of the consideration for this Agreement 4. INSURANCE. 6.1 Time f or C ompli a n ce . As a c ondition p rece d e nt to the e f fec tiv e n e ss of this A g r e e m e nt f or w o r k to be p erf o r m e d h e re und e r a nd without limiting the ind e mni t y p r ovisions of the A g r ee m e nt, the C onsult a nt in p ar ti a l p e r f o r m a n c e o f i t s obli g a tions und e r su c h Ag ree m e nt, s h a ll p r o c u r e a nd m a int a in in f ull f o r c e a nd e ff e c t du r i n g the t e r m of the A g ree m e nt, the following policies of insurance: 6.2.1 Commercial General Liability: Commercial General Liability Insurance which affords coverage at least as broad as Insurance Services Office “occurrence” form CG 0001, with minimum limits of at least $1,000,000 per occurrence. Defense costs shall be paid in addition to the limits. This Agreement shall be binding on the successors and assigns of the Parties. 11.11 Amendment; Modification.
