75C Diamond Valley Road, Markleeville, CA 96120 (530) 694-1816 / Fax (530) 694-2387 Web Page - http://www.alpinecountyca.gov Gail St.James Director Behavioral Health COUNTY OF ALPINE Behavioral Health REQUEST FOR PROPOSALS Yoga Classes Fiscal Year 2019 – 2020 Introduction Alpine County Behavioral Health Services (ACBHS) has been providing yoga classes to residents of Alpine County in Markleeville for the past few years as part of the Mental Health Services Act (MHSA) plan. The MHSA represents a statewide movement to provide a better coordinated and more comprehensive system of care for those with serious mental illness, and to define an approach to the planning and the delivery of mental health services that are embedded in the Mental Health Services Act Values: Wellness, Recovery, and Resilience; Cultural Competence; Client and Family Driven Services; Integrated Service Experience; and Community Collaboration. The MHSA provides funding for Prevention and Early Intervention (PEI) services, as well as for Community Services and Supports (CSS) that are focused on addressing the needs of unserved and underserved populations and providing integrated services that support resilence and recovery from mental illness. Alpine County is a unique county in that it is the smallest county by population in the entire state of California (roughly 1100 people total), spread out over 743 square miles, in 5 distinct communities. This equals out to 1.6 persons per square mile. Alpine County is considered a “frontier county”. To continue to provide yoga classes to residents of Alpine County, ACBHS is soliciting proposals from certified yoga instructors at a rate of $375 per class. Scope of Work Consultant will provide Wellness and Yoga Services for the benefit of the residents of Alpine County in Markleeville. The classes will take place on Tuesday and Thursday evenings in Markleeville for 90 minutes (5:30-7:00pm) year around (including during storms throughout the winter), excluding holidays. Yoga participants come regularly and classes can range from 2 to 18 participants depending on a variety of factors (weather, time of year, personal lives, location, etc). Task 1 Provide yoga classes for residents that are inclusive for all fitness levels and abilities. ---PAGE BREAK--- Task 2 Consultant responsible for the security and control of all County owned equipment and facilities used. Task 3 Consultant will be responsible for providing class sign in sheets (template provided) with billing at the firstof the month to MHSA Coordinator. Task 5 Consultant is required to collect participants contact information and notify them of schedule or location changes. Task 6 When consultant is going to miss a class, we request you provide a substitute and proof of their insurance. Schedule Classes will occur July 2019 thru June 2020 in Markleeville. Proposal Requirements The proposal for consultant services must include, at a minimum, the following: 1. Yoga Certification 2. Proof of Liability Insurance, including coverage for sexual misconduct in regards to minors 3. Consultant/Company Profile Information. This shall include the consultant/company name, street address, telephone & fax numbers, e-mail address, list of principals and/or owners, and general consultant/company background information. 4. List of all substitutes proposed to be involved in the provision of consultant services and their qualifications. 5. List of three professional references for the consultant/company. 6. Consultant must make a minimum of one year commitment or longer as the consistency of the teacher greatly affects class regularity and attendance. 7. Eligibility and Application Guidelines 1. Applicants from all geographic areas of Alpine County and surrounding areas are strongly encouraged to apply. 2. Applicants may submit only one proposal per organization or entity. Funds may not be used to supplement existing programs. 3. All applications will be subject to a review process for rating. The review panel will consist of: ACBHS Director, MHSA Coordinator, ACBHS Coordinator, Alpine County CAO, and County Counsel. ---PAGE BREAK--- 4. According to the Alpine County Standard Agreement, all applicants must show proof of commercial liability insurance, automobile liability insurance, worker’s compensation, and professional liability insurance. 5. The Board of Supervisors reserves the right to modify or reject any and all bids. How to Apply Scope of Work: Describe experience and commitment to providing yoga classes in Markleeville. Justification: Detail ability to meet all tasks and requirements listed herein. Submittal of Proposal All proposals must be submitted no later than June 28, 2019 4:00p.m. to the Alpine County Clerk’s Office at: Alpine County Behavioral Health Services c/o Alpine County Clerk 99 Water Street P.O. Box 158 Markleeville, CA. 96120 Any incomplete proposals or proposals submitted after the submittal date shall be rejected. The proposal may be completed electronically, but must be printed and signed for submittal. Fax and postmarks not accepted. Please be aware that due to our location, overnight delivery services may not be reliable. Please submit one signed original proposal and two copies. Selection Process The review panel will select the most qualified consultant that meets the County’s needs. The project will be awarded to the selected consultant.
