You must be doing your own work. • (print name) owner and occupant of the address listed below recognize that it is my responsibility to follow Building Code and that I have reasonable knowledge of the code to successfully complete the project in which I am obtaining permits for. My failure to meet standards required by code may result in permit cancellation, the need for an Architectural Engineers letter and/or a “tear out” of work completed. • Must prove ownership of said property. • Must live at the property for one year following completion of the job. • Cannot hire anyone to do work under a self permit.
