RFQ 2021-01 ADDENDUM #3 January 21, 2021 The following information is provided as a result of question(s) posed by vendor(s): QUESTION 1: The submission requires a “non-collusive proposal certification and acknowledgement form” as outlined in section 16. This form certifies a price proposal, which is not being submitted with these qualifications. Do you still require this form despite it not being tied to a cost proposal rather than submitting it with a cost proposal in the future? ANSWER 1: While no cost proposal is solicited at this time, the City nevertheless expects to see the non-collusive proposal certification and acknowledgement form from each of the Proposers.
